Greetings and welcome to the Meadowdale family, Home of the Meadowdale Lion Cubs!  We believe that the school staff and our families are a viable team working together to assist our children in their academic, social and emotional growth.  We hope that you will find this site to be helpful. We are dedicated to providing a sound educational program for you and your child. We extend our best wishes for a great school year.

 

    What is needed for school?

Each school year the registration form and emergency forms must be completed. 
In addition, when registering a student for school,  parent/guardian(s) should bring the following verifying documents:
  • Photo Identification with signature
  • Child's Birth Certificate / Proof of Birth
  • Child's Shot Records    Kindergarten Only: 4 DPT or DTaP, 3 Polio, 2 MMR, 3 Hepatitis B and 1 Variecella 
  • Custody Documentation (if applicable, divorce, foster-care or adoption decree)
  • Child-care Provider's name, address and phone number
Is it time for Kindergarten?  Yes, if your child will be 5 years old on or before September 30th.
