Student Records

Release of Records Form

A signed release and picture ID are required prior to releasing any student’s school record.

Once a student reaches 18 years of age, only the student can authorize release of his/her record (ORC3.22). This means that parents/guardians of students who are 18 or older cannot request or receive their student’s record without written authorization or a Release of Records form signed by the student..

Government agencies such as penal institutions, Social Security Administration, and Adult Parole Authority are not assessed a transcript fee. Anyone else must pay the $5.00 fee (cash, cashier’s check or money order) before receiving a student’s record. Do NOT send cash through the mail.

Graduate Records

High schools that are open maintain records for their graduates. Records for high schools that have closed (Fairview, Kiser, Stivers, Roth, Roosevelt, Wilbur Wright and Steele) are kept on microfilm in the Central Records Office. Requests for these records should be directed to Central Records, 542-3296.

Non-Graduate Records

All schools maintain records for their current students. A student’s record follows them to each Dayton Public School building they attend.

Open schools also maintain records for students who are not attending, who have left Dayton Public Schools, or non-graduating seniors.

Closed Schools

Records for schools that have closed are located in the Central Records Office on the 2nd floor of the Administration Building. Requests for these records should be directed to Central Records.

*Stivers-Patterson:

Stivers-Patterson High School was formed in 1973. Students went to Stivers for 9th grade then to Patterson for grades 10-12. Some of the records were recorded on microfilm and are with the Stivers records in Central Records and some are at Patterson. Former Stivers-Patterson students should first contact the Central Records Office to request these records; if Central Records does not have them, you will be referred to Patterson.

RECORDS QUESTIONS AND ANSWERS

Q: Where are records located?
A: Records for closed schools are at Central Records.
Records for schools that are still open are at the schools no matter how old the record.
Some inactive records for students born 1/1/70 or later are at Central Records.


Q: How can I get my records?
A: In person:
Come to the Student Assignment Office with a picture ID.
Complete a Release of Records form; provide $5.00 cash, cashier’s check or money order.

By mail:
Mail signed release, a copy of your picture I.D. that includes your signature, and $5.00 cashier’s check or money order to:

Release of Records form should include: student’s complete name at time of attendance, date of birth, name of school and approximate dates of attendance, signature and return address.

Q: Can I be billed for the record?
A: No. Payment must be received before the transcript is released.


Q: Can I get a copy of my husband’s (child’s/parent’s) record?
A: No. The 1974 Privacy Act prohibits the release of a record without the authorization of the individual.

Q: Will you accept a faxed release?
A: Yes, if all that’s required is verification of attendance/graduation.
If a copy of a transcript is required, payment must be received prior to its release. Obviously, money orders/cash/business checks cannot be faxed.

Q: Can you tell me over the phone if someone graduated from XXXX High School?
A: Yes. The requester must first provide a valid release for educational records from the student. Verification can be made over the phone or fax.

Q: I am doing a genealogy search. Can you tell me about my relatives?
A: Yes. The requester needs to provide a written request and $5.00. The request should contain the name of the individual being researched and the school he/she attended. While it is not possible to provide a copy of the transcript because of the Privacy Act, we can provide directory information which includes the grades the student was in the school, their parents’ names, their home address at the time they were in school, date of birth, and place of birth. All of this information is contingent upon its being recorded on the student record, of course.

Q: Can you fax my transcript?
A: Records are kept on microfilm and do not fax well. A faxed copy would be a picture of a picture of the original record and is usually illegible after being faxed.
