A special “Get Enrolled and Ready” event will be held at the DPS School-Based Health Center for children ages 3-6 who need to enroll in the Dayton Public School District for next year. The event is free and is designed as a one-stop shop for enrolling young students.

Date: Monday, June 27th

Time: 1-5 p.m.

Location: Dayton Public School-Based Health Center (Roosevelt Elementary School, 1923 W. Third Street)

Those who attend will receive:

• Registration and enrollment assistance
• Physical exams
• Immunizations and vaccines
• Vision screenings
• Dental screenings
• Health insurance enrollment assistance

Parents and guardians must bring the following to the event:

• Child’s birth certificate
• Proof of residency
• All shot records
• Custody/guardianship documents
• Current insurance card (if any)
• And, other important paperwork to help us get your children enrolled and ready!
