COVID-19 Vaccine Clinics Available for Students ages 16-19 and Families

The Dayton Public School District is partnering with Dayton Children’s Hospital to hold COVID-19 vaccine clinics for DPS students ages 16-19 and their families beginning in May. 

How?

  1. Fill out a consent form: In order for students under 18 to be vaccinated, parents or guardians must print, complete and sign the consent form. This form must be brought to the appointment for the student to be vaccinated.
  2. Sign up for a spot: Anyone who wishes to receive a vaccine must sign up online. Sign up for a spot here: https://www.childrensdayton.org/school-covid-19-vaccines
  3. Bring your identification: Students should bring a DPS student ID, state ID, or driver’s license.

Who?

DPS students ages 16-19 and their families

What?

Clinics will only offer the Pfizer vaccine, as it is currently the only vaccine approved for those under 18. A second dose of the Pfizer vaccine will be provided three weeks after the first dose. 

When?

Clinics will be held at Belmont High School and Thurgood Marshall STEM High School between 3:30 p.m. and 5:30 p.m. You must sign up.
