REQUEST FOR TRANSCRIPT/RECORDS FORM
All inquiries including graduation verification, transcript, shot records, IEP, ETR, and cumulative folder must be requested online using our new online student record request system. Fax, phone calls and mail will no longer be accepted.
Students who have graduated from Dayton Public Schools may request a duplicate diploma and/or diploma cover. Transcript processing time may vary from 10 - 30 business days.
All schools maintain records for their current students. A student's record follows them to each Dayton Public School building they attend.
Open schools also maintain records for students who are not actively attending Dayton Public Schools. Each building only keeps 5 years of records in their building. Requests for those records should be directed to the respective school.
High schools that are open maintain records for their graduates. Each building only keeps 5 years of records in their building. Requests for those records should be directed to the respective school.
CLOSED DPS AND CHARTER SCHOOLS
Records for schools must be requested online. In the case of charter schools that have closed, the district can only supply records that were provided to the district upon the school's closure.
Records for closed schools are the responsibility of Central Records.
Records for schools that are still open are at the schools. Schools only maintain 5 years of records.
Some inactive records for students born 1/1/70 or later are managed by the Student Enrollment Center.
