Gather the required documents in order to enroll your students at Dayton Public Schools. The list is as follows –
- Only a parent or legal guardian may register a student. A legal guardian must provide legal documentation of guardianship
- Child’s birth certificate (or child’s passport if the parent(s)/guardian(s) name is included)
- Child’s shot records
- Proof of Address (see below for acceptable proof)
- Proof of grade level (report card, progress report, transcript, withdrawal form)
- Custody documentation (If applicable. Must be court-stamped and signed by a magistrate or judge, or custody given by the accredited agency. The state of Ohio does not recognize a notarized statement for custody.
Acceptable Proof of Address Documents (October 2020)
Primary AND Secondary Proof of Address must include the custodial parent/legal guardian’s first and last name and complete address. Documents must be dated within 60 days of the appointment.
PRIMARY PROOF OF ADDRESS
- Lease Agreement (must include landlord and tenant signatures)
- Mortgage Statement (dated within 60 days)
- Deed
- Proof of Purchase
- Property Tax Bill
SECONDARY PROOF OF ADDRESS
No secondary proof of address is needed if the Lease Agreement is issued within 60 days.
- Utility Bill (Water, Electric, Gas, Propane, Cable)
- Paycheck Stub
- Court Document
- Change of Address
- Documentation on Employer’s Letterhead verifying the address
- Job & Family Services Documentation
- Social Security Statement
- Shelter Letter
