Register children for preschool at annual DPS Preschool Fairs

WHAT: DPS Preschool Fairs
WHEN: Tuesday, February 25, 2020, and Wednesday, February 26, 2020 at 4:30 p.m. – 6:30 p.m.
WHERE: Roosevelt Elementary, 1923 W. Third St., Dayton, OH 45417

INFO: Parents of children ages 3.5 to 4 years old can enroll them in free, 5-star preschools in the Dayton Public Schools. Free physicals and dental exams for children currently in preschool, or children registering for preschool, will be available at the DPS school-based health center during the preschool fairs. 

Can’t make it? Physical and Dental Exams Available via Walk-Ins and Appointments:

  • March 3rd & 4th from 4:30 p.m. – 6:30 p.m.
  • March 10th & 11th from 1:00 p.m. – 3:00 p.m.

To register a student, parents will need:

  • The child’s birth certificate
  • The parent’s valid photo ID
  • Proof of residency*
  • Completed shot record (must follow the state immunization schedule)
  • Legal custody documentation (if applicable)
  • Medical form (dated within the last 12 months)
  • Dental form (dated within the last 12 months)

​Parents who are interested in enrolling their child at River’s Edge Montessori cannot sign up at this fair. Please call the school directly at 937-542-4640.

*Effective August 1, 2019, all Parent/Guardian(s) must provide a Primary AND Secondary Proof of Address for Enrollment and Change of Address.  Listed below are the acceptable documents.

All documents listed below must include the custodial parent/legal guardian’s:

  1. First and Last Name
  2. Complete Address
    1. Dated within 60 days of parent’s visit to the Student Enrollment Center or Assigned School
    2. NO Secondary Proof of Address is needed if the Lease Agreement is issued within 60 days
    3. Acceptable Documents
      • PRIMARY PROOF OF ADDRESS
        1. Lease Agreement (must include landlord and tenant signatures)
        2. Mortgage Statement (dated within 60 days)
        3. Deed
        4. Proof of Purchase
        5. Property Tax Bill
      • SECONDARY PROOF OF ADDRESS (must be within 60 days)
      1. Utility Bill (Water, Electric, Gas, Propane, Cable
      2. Paycheck Stub
      3. Court Document
      4. Change of Address 
      5. Documentation on Employer’s Letterhead verify address
      6. Job & Family Services Documentation
      7. Social Security Statement
      8. Shelter Letter
      9. Parent’s valid photo I.D. that includes his/her signature (must not be expired)
